To add to what J-me said Fungus Clear should help with the split tails if it's caused by tail rot.

Otherwise it sounds to me like it's maybe the Nitrites. Do you know what those levels are or are they the ones meant by the nitrate posting. They look like the levels suggest NitrItes. If so a 25%-30% water chang eis needed.

You said you've had the tank for 2 weeks, Have you done any water changes yet? If not it's time to do one. Then keep up on your weekly changes.

Uncle Scott wrote a really interesting reply to one of my diary questions about water changes and how much we should be changeing every week. It sounds like twice a week 25% water changes (50% water changed in a week) are encouraged by some prominent people in the fish world. Here's the link if you want to check it out http://www.guppylog.com/story/2007/2/27/85712/1920

If the "white spot" sickness was Ich then the treatment for it wasn't long enough. I've read that Ich/Ick should be treated for a week or two after all the spots disappear. Were the white spots looking like someone had salted the fish or dipped it in sugar? That's what Ick kinda looks like (most of the time). If it just looks like white puffy bumps then it could be some type of fungus and the hemorrhagic septicemia is a secondary infection. Fungus Clear should help with both of those conditions. If using fungus clear make sure to remove any carbon from the filter and any biofilter that may be in the filter box. Also you may need to re-cycle the tank after treating with the fungus clear.

Good luck and let us know how things go. Also next time post a diary. They get noticed sooner and thus people respond quicker.
Before all else fails....do a 25% water change ;)
