The appearance of an internal blood red spot such as described very likely indicates an internal rupture of tissue and internal bleeding. Lots of causes but bacterial problems are most likely and secondarily intestinal parasites.

Best to destroy the fish as soon as possible. Trying to keep the fish alive without some quarantine vessel only promotes the possibility of spreading the problem around to the other tank mates.  The likelihood of the fish surviving is extremely small considering the enormous size of the observed spot compared to the size of the body (the guppy) affected.

I would strongly suggest the purchase of some extra tank space though and a way to quarantine future tank mate prospects. One would hate to loose the entire tank just because there was an unforgettable fish at the LFS.
